1About this consent
This document explains telehealth care provided through Second Season and the independent providers we work with. When you begin an assessment you will be asked to give your informed consent to receive care this way. This page is the information behind that consent.
2What telehealth is
Telehealth is health care delivered using secure technology instead of an in-person visit. Depending on your state, a provider may review your information through a secure online assessment, or a live video or phone consultation may be required first. Your provider uses the information you give, and any information they request, to make clinical decisions.
3Benefits
- Access to a licensed provider without travel or a waiting room.
- Care and written guidance you can review again at home, on your own schedule.
- Privacy and convenience for sensitive concerns.
4Risks and limitations
- Without an in-person exam, a provider may not detect a condition that a hands-on visit or testing would find.
- Care depends on the accuracy and completeness of the information you provide. Missing or inaccurate information can affect the plan and its safety.
- Technology can fail, and information sent electronically could, in rare cases, be disrupted or accessed without authorization despite safeguards.
- Telehealth is not appropriate for every condition, and your provider may tell you to be seen in person.
5Your provider relationship
Your care is provided by an independent, licensed provider, not by Second Season. The provider exercises independent professional judgment, may ask for more information, and may decline to prescribe if treatment is not appropriate for you. You may seek in-person care at any time, and you may ask for a referral.
6Prescriptions and compounded medications
If your provider prescribes treatment, it may include a compounded medication prepared by a licensed pharmacy. Compounded medications are not FDA approved and are not reviewed by the FDA for safety, effectiveness, or quality, and they are not generic versions of or equivalent to any brand-name drug. Your provider will review the risks, benefits, and alternatives with you, and all medications carry possible side effects.
7No guarantee of results
Medicine is not exact, and people respond differently. No provider and no treatment can promise a specific result. Your outcome depends on many factors, including your health and how closely you follow your plan.
8Your responsibilities
- Give accurate, complete, and current information about your health, medications, and allergies.
- Follow the plan your provider gives you, and do not change doses on your own.
- Report side effects, new symptoms, or concerns to your care team promptly.
- Keep your own physician informed, and continue routine care and screenings.
9Alternatives
You may choose in-person care with your own physician or a local clinic instead of telehealth at any time. Choosing telehealth now does not prevent you from seeking in-person care later.
10Privacy and records
Your care generates a medical record that is protected under HIPAA and handled under the provider's Notice of Privacy Practices. Our handling of your information is described in our Privacy Policy. You may request a copy of your records from your provider, who is the custodian of that record.
11Emergencies
Telehealth is not for emergencies. If you are having a medical emergency, call 911 or go to the nearest emergency room. Do not wait for a response through this service.
12Controlled substances
We do not prescribe controlled substances through our online assessment. If a provider believes a controlled substance is appropriate for you, you will be directed to in-person care. [CONFIRM CONTROLLED-SUBSTANCE STATEMENT WITH PROVIDER GROUP]
13Voluntary consent and withdrawal
Your consent to telehealth is voluntary. You may withdraw it at any time and stop using the service, without affecting your right to seek care elsewhere. Withdrawing consent does not undo care already provided.
14Electronic communication
By using the service you agree to receive communications about your care and orders electronically, including by email and secure message. You can ask questions of your care team through the service, and you can opt out of non-essential marketing messages at any time.
15How your consent is recorded
When you begin your assessment, you will be asked to confirm that you have read and understood this consent and that you agree to receive care through telehealth. Your agreement is recorded with the date and time and kept with your record.
